Before you are treated by one of our providers, you will be asked to give us some information.  If you'd like, you may click and download the form above and bring it with you to your first visit.

You will be asked to present your 

insurance card(s) at each visit.  

This is to ensure that we file your medical claims to the appropriate insurance company.   

   All patients are responsible for getting a referral to our office from their insurance company and/or primary care physician (if needed). 

   Please contact your insurance company before your first visit, to ensure that we participate with your insurer and that you will not be totally responsible for your account balance. 

OUR BUILDING AND PHILOSOPHY

Our building is designed to be comfortable and professional.  Because of this intent, we do not discuss financial or insurance matters in the reception area.  For your privacy, personal matters are discussed one-on-one in an exam room with a Patient Account Representative.

Our patient account representatives will make every effort to help you understand any insurance or payment circumstances of your visit.  They will collect your co-payment (if your insurance company requires that you pay a co-payment) and verify insurance numbers, referral numbers, pre-authorizations for services, and demographic information.  If you have any questions during the course of your treatment, our patient account representatives or business office will be happy to assist you!
